Q-Codes: Temporary HCPCS Codes Explained

Q-codes are temporary HCPCS Level II codes (Q0001–Q9999) assigned by CMS when a service, drug, or item needs to be billable before it receives a permanent code assignment. These codes cover a wide variety of services including certain diagnostic tests, casting supplies, infused drugs, flu and pneumonia vaccines, and hospice care components. The "temporary" designation means CMS intends to eventually replace the Q-code with a permanent code from another section — or may delete it if the item is no longer covered. Some Q-codes have remained in use for years while awaiting permanent classification. Billers should not assume a Q-code will be discontinued; instead, track its status through the quarterly HCPCS update log.

Q-codes for vaccines are particularly common in physician office billing. Influenza vaccines, pneumococcal vaccines, and hepatitis B vaccines each have specific Q-codes that correspond to the product type and patient population. The correct Q-code must match the actual vaccine administered — using the wrong code, even for a similar vaccine, can result in a denial.

For drugs that are temporarily covered by Q-codes, the same NDC reporting requirements apply as for J-codes. Billers should use the NDC lookup tool to confirm the crosswalk between the drug product NDC and the appropriate Q-code before submitting a claim.
