Physician Fee Schedule

The Medicare Physician Fee Schedule (PFS) sets payment rates for services provided in a physician office or outpatient clinic. Each code has a facility rate (when service occurs in a hospital or ASC) and a non-facility rate (when in a physician office). The non-facility rate is typically higher because it includes practice expense overhead.
