LT

Overview

Modifier LT identifies a procedure or item furnished to the left side of the body. It is most commonly used with anatomically-specific HCPCS codes — particularly orthotics, prosthetics, and certain DME items — to distinguish which side received the service. For bilateral procedures, LT and RT are used together (or a separate bilateral modifier may be appropriate).

When to use

• The procedure or item is anatomically left-sided.
• The HCPCS code does not already specify laterality in its description.
• Required by the payer policy for the specific code and date of service.

Common mistakes

• Using LT for procedures that are inherently bilateral and don't require side designation (e.g., midline procedures).
• Forgetting to add LT on orthotic codes — many L-codes deny without an explicit laterality modifier.
