E-codes (E0100-E9999) are the workhorse codes of the DME industry. They cover walkers and canes (E0100-E0159), hospital beds (E0250-E0329), wheelchairs (E1130-E1298 for manual, K-codes for power), oxygen and respiratory equipment (E0424-E0488), CPAP/BiPAP (E0470-E0601), and hundreds of other home medical devices.
E-code billing nearly always requires DMEPOS modifiers — NU (new equipment), RR (rental), UE (used equipment), KH/KI/KJ (rental month indicators), and KX (documentation on file). Coverage depends on a face-to-face encounter, a Standard Written Order, and item-specific medical necessity documentation per the Local Coverage Determination (LCD) for the supplier's jurisdiction.
