J-codes (J0120-J9999) identify drugs administered by any route other than oral — intravenous, intramuscular, subcutaneous, intra-arterial, inhalation, and most other delivery methods. Each J-code specifies a generic drug name and a unit of measure (e.g., J0129 is abatacept 10 mg, J9035 is bevacizumab 10 mg).
J-code pricing is unique among HCPCS codes: Medicare pays based on the Average Sales Price (ASP) reported by manufacturers, plus a small administrative percentage (currently 6%). ASP rates are published quarterly and refreshed every January, April, July, and October.
The NDC-to-HCPCS crosswalk maps every National Drug Code (NDC) approved by the FDA to its billing J-code. When a provider buys a vial of Orencia (NDC 00003-2188-11), they bill J0129 × the number of 10 mg units administered. The crosswalk is essential because clinicians and pharmacies think in NDCs, but Medicare and Medicaid claims processors only accept J-codes.
