P-codes (P2000-P9999) cover laboratory and pathology services, including blood products (P9000-P9099) and chemistry tests not represented in CPT. Most P-codes are paid under the Clinical Laboratory Fee Schedule (CLFS), a national fee schedule with no facility/non-facility split.
P-codes are typically billed by independent labs, hospital outpatient labs, and physician office labs. The most heavily-used P-codes are P9011 (transfer agent) and P9012 (cryoprecipitate).
