L-codes (L0100-L9900) cover the orthotic and prosthetic (O&P) field. L0100-L4999 are orthotics (back braces, knee braces, foot orthotics, cervical collars, scoliosis braces). L5000-L9900 are prosthetics (artificial limbs, prosthetic components, terminal devices). L-codes are dominated by the DMEPOS fee schedule.
O&P billing requires meticulous documentation: a detailed written order, proof of medical necessity tied to a specific ICD-10 diagnosis, often a wear-time log, and adherence to LCDs for the supplier's region. Many L-codes require modifier KX, and certain prosthetic codes require modifier RT or LT to identify the affected limb.
